Additional Toolkits and Information

Working together with our partners the Million Hearts Collaboration helps to promote resources and toolkits developed to help strengthen work in communities, hospital systems and in clinical practices.

Below are a number of toolkits released as a part of The Million Hearts® initiative, these publications and toolkits can be utilized to advance your heart disease and stroke prevention programs and treatment efforts and support Million Hearts® goals in your everyday work.

  • Link to the guide and details about the The Million Hearts® Collaboration Community-Clinical Linkages Toolkit  
  • Million Hearts® in Municipalities Toolkit - The Centers for Disease Control and Prevention (CDC) teamed up with the National Association of City and County Health Officials (NACCHO) to develop the new Million Hearts® in Municipalities Toolkit, a tool to help health departments and municipal organizations implement Million Hearts® at the local level. It was designed to engage city, county, and regional partners to adapt the Million Hearts® structure and framework to the needs and assets of their community. It includes 5 modules that can be used, individually or as a package, to tackle capacity needs related to goal-setting, partnerships, communications, and evaluation and monitoring. Each module includes a list of online resources that may be used to inform the design of a municipal’s evidence-based approach to prevent heart attacks, strokes, and other cardiovascular events. We encourage you to explore how this tool may be used to inform your cardiovascular disease prevention efforts.
  • For a model local health department and community toolkit for improving hypertension outcomes, especially in smaller and more rural communities, review the Set Your Heart on Health publication from Million Hearts WI and the Association of State and Territorial Health Officials (ASTHO) Million Hearts Learning Collaborative.
